Part A & B Forms

The Part A Form is called "Informed Consent, Release Agreement, and Authorization" and is one page long.  All Pack 68 participants, including scouts and family members who regularly attend scouting events, need to provide us with a signed copy of a form called Part A.  When completed, you may email it to CommitteeChair@pack68union.org or hand it into your Den Leader at your first meeting.  

The Part B Form is called "General Information/Health History" and is two pages long.  All Pack 68 participants, including scouts and family members who regularly attend scouting events, need to provide us with a copy of this form.  Please do NOT email it to us.  It will be collected by your Den Leader in person.  It is a requirement for all ranks to review this form with your Den Leader on at least an annual basis to make sure any necessary updates are recorded.
